In order to register and report earnings, you will need a plan code. If you don’t have one, please submit a request.

Although custom reports can be uploaded into the Contributions Manager, downloading and using our templates or changing the existing category titles in your reports may save you time when uploading. Download the template to be used by broadcast employers or all other employers.
